I want to draw this shape flat on a piece of paper and then fold into the shape.

Any ideas? I know the measurements of the two circumferences, can work out the heights and angles of the finished article, but the bottom will be curved to produce the conical type shape when folded, and the top will almost be arch shaped I think. Also the drawn angle will be different to the finished??
